Facility Accreditation Application Submissions

How to Submit Facility Accreditation Application Documents, Scans and Reports

Copies of your supporting documented items, such as lists of CMEs/CEUs, licenses, certificates, forms, graphs, records, logs, Interpreted Reports, etc., can be submitted in electronic formats such as Microsoft Word, PDF, GIF, JPEG, PNG, etc. The key to submission is high resolution.

DXA Scans must be submitted as source files or originals. Photographed copies and faxes are not accepted. E-mails of submissions will not be accepted.

Every scan and interpretive report must be HIPPA compliant. This means that no patient specific information (i.e., name, social security number) can be on the submission. Instructions for making scans HIPPA compliant are available on the Frequently Asked Questions page of our Web site.

In an effort to send the electronic submissions to ISCD for accreditation and to make it easy as possible for your facility, the Facility Accreditation Council suggests the following methods:

“Sneakernet” 
If you have the appropriate manufacturer software to do this, take source, originals or raw data files directly off your DXA. Copy the scan files directly off of the DXA machine onto a “USB thumb drive” or memory stick and walk the thumb drive over to the computer that has the accreditation application on it. Plug the thumb drive into the UBS port on the back or front of the computer. Create a folder on your hard drive. Move the scans into this folder. When ready, hit the “Browse button” found on the accreditation application and browse to the folder you created and named and upload the folder contents.
 
2.
JPEG 
Electronically convert the original DXA scans to JPEG files. The JPEGs must be at least 60 KB for the best resolution. Copy them to the hard drive folder that you have created. When ready, hit the “Browse button” found on the accreditation application and browse to the folder you created and named and upload the folder contents.
 
3.

CD
Put all DXA scans on a CD and insert the CD into the computer that has your accreditation application on it. Copy them to the hard drive folder that you have created. When ready, hit the “Browse button” found on the accreditation application and browse to the folder you created and named and upload the folder contents.

 
4.

Document Scanner

Place original print-outs of the source DXA scans on your document scanner. Set the default to 150 DPI. Scan the print-outs to the folder you have created on the hard-drive. When ready, hit the “Browse button” found on the accreditation application and browse to the folder you created and named and upload the folder contents.
