This will help referring healthcare providers to determine whether
a bone density test is likely to be a covered benefit and help bone
densitometry centers in billing Medicare, insurance companies and
health plans.

- Names of the individual state health care carriers,
- Telephone numbers and e-mail addresses of those carriers,
- Criteria set by the carriers for performance of bone density testing,
- Appropriate procedural codes for the specific bone density testing device used in the office,
- Diagnostic codes for the disease process in which the procedure is being utilized, and
- Comments that will further assist the office when interacting with carriers.
